  • New registration certificate in your name

If the registration is not in your name, also attach one of the following documents: 

  • Vehicle lease 
  • Declarations page from your insurance policy or letter from your employer on company letterhead, providing a description of the company vehicle (make, model and year) and naming you as the primary driver
  • If you have a green license plate, also provide a colour photo of it.
  • And one of the following documents: 
    • Old parking sticker
    • If you have sold or scrapped your vehicle or changed your license plate number, the transfer document from the Société de l’assurance automobile du Québec (SAAQ).
    • Document from the dealer showing that they took back the vehicle, if you traded it in
    • Ownership Transfer and Power of Attorney Document for Stolen or Totaled Vehicles, Société de l’assurance automobile du Québec (SAAQ)
    • SPVM Incident Report, in the event of a theft
    • Total Loss Vehicle Declaration provided by the insurance company

Proof of residence

Please attach one of the following documents, dated within the past 3 months and showing your name and address:

  • School or municipal tax bill
  • Utility bill (Hydro-Québec, gas, telecommunication company such as Bell, Vidéotron, Rogers, or other) 
  • Invoice from an educational institution
  • Monthly statement from a credit or financial institution
  • Valid driver’s license

Leases are not accepted as proof of residence.

  • Québec health insurance (RAMQ) card
  • Valid driver’s license
Entrepreneur and own the vehicle (if so)

Has your car broken down, or been damaged in an accident or stolen? Get a sticker that is valid for one month to park a vehicle on loan from the garage or your insurer.

2. Gather the necessary documents

You must attach the following supporting documents to your application, depending on your situation.

Change of vehicle or registration

  • New vehicle’s registration
  • Old parking sticker or one of the following documents:
    • If you have sold or scrapped your vehicle or changed your license plate number, the transfer document from the Société de l’assurance automobile du Québec (SAAQ).
    • If you have traded in your vehicle, a document from the dealer showing the trade-in

Lost, stolen or damaged sticker

  • A piece of ID 
  • Damaged sticker
  • In cases of theft, the police report or incident number

Courtesy car

  • The vehicle’s registration certificate and the rental documents from the insurance company or garage that state that a courtesy vehicle is on loan

And one of the following documents:

  • SAAQ accident report or, in case of theft or vandalism, the SPVM incident number
  • The repair authorization or a work order from the garage proving that your vehicle is under repair
  • An estimate by the insurance company’s assessor of the damage caused to the vehicle
