Forms of ID and proof of address that are accepted at points of service

You must show proof of ID and/or address for certain services and permit applications. Here are the documents that are accepted at all of our points of service.

Accepted forms of ID

  • Citizenship card or certificate
  • Québec driver’s license
  • Québec health insurance card
  • Passport
  • Short-form or long-form birth certificate

Accepted proof of address

Your proof of address must be dated within the past three months.

  • Bill from an educational institution showing your name and address
  • Bill from a public utility (Hydro-Québec, gas or telecommunications company such as Bell, Videotron, Rogers or other provider) showing your address
  • Correspondence from the municipal court
  • Monthly statement from a credit or financial institution
  • Monthly statement of investments from a trust or financial institution
  • Notice of change of address from Canada Post or proof of mail forwarding (sticker)
  • Québec health insurance card renewal notice
  • Record of Employment Insurance
