Forms of ID and proof of address that are accepted at points of service

You must show proof of ID and/or address for certain services and permit applications. Here are the documents that are accepted at all of our points of service.
Accepted forms of ID
- Citizenship card or certificate
- Québec driver’s license
- Québec health insurance card
- Passport
- Short-form or long-form birth certificate
Accepted proof of address
Your proof of address must be dated within the past three months.
- Bill from an educational institution showing your name and address
- Bill from a public utility (Hydro-Québec, gas or telecommunications company such as Bell, Videotron, Rogers or other provider) showing your address
- Correspondence from the municipal court
- Monthly statement from a credit or financial institution
- Monthly statement of investments from a trust or financial institution
- Notice of change of address from Canada Post or proof of mail forwarding (sticker)
- Québec health insurance card renewal notice
- Record of Employment Insurance
