Are you being asked for a certificate of life or a certificate of residence in Montréal? In most cases, you must declare under oath that everything stated in the certificate is true.

A certificate of life and a certificate of residence are documents issued by an institution in your country of origin. You may need them in order to receive pension payments.

To make your sworn declaration, visit an Accès Montréal office after completing the form you received. You must sign it on site in the presence of the commissioner of oaths.
